Introduction

Our study compares the outcomes of the INNFOCUS® Glaucoma Filtration Device (Alcon, Fort Worth, TX) implant observed in POAG patients for 2 years with those of patients undergoing trabeculectomy.

Patients et Methodes

The subjects comprised sixteen eyes of sixteen cases with open-angle glaucoma for which filtration surgery using INNFOCUS® was performed by two operators from June 2014 and observed for at least 1 year (Innfocus® group), and sixteen eyes of sixteen cases for which trabeculectomy was performed by the same operators (TE Group), in the same period. All cases were consecutive, and were observed for at least 1 year. Their background factors were comparable to those of the innfocus group. For both groups, mitomycin C 0,02 was used .

Résultats

No significant difference in IOP was observed at 1 month after surgery : 9,56 + /- 2,30 in Innfocus group vs 10,06 +/- 3,51 in TE group at 1 Month p = 0,63. Results at 6 months show a significant difference with an increase in IOP in the Innfocus arm without a significant difference in number of treatments at 6 months . At 1 year no significant difference in IOP was observed :14,7 +/- 2,8 vs 14,1 +/- 3,7 (p=0,27). 91% of therapeutic reduction was observed in trabeculectomy group vs 63% in innfocus group.

Discussion

Hypotony and choroidal detachment were observed as early postoperative complications during one to two-week period in nearly 20 % of the cases in the TE Group, while there was no early or late hypotony in the innfocus group. Hyphema was observed in 2 patients in the Innfocus group, none in the TE group. The Innfocus Group had fewer post operative management, whereas in the TE group there was 10 LSL and 3 leaking blebs to handle. Surgery duration was significantly shorter in the innfocus group.

Conclusion

Filtration surgery using Innfocus device is a novel, fast and easy type of glaucoma surgery. Partial results tends to show an increase post-operatively with a weak response to bleb needlings and antimitotic drugs.
